Possible: A number has been ported via ICMS or ipro but no web port form has been generated. If you have raised a manual form then please send it in to Upside once the transfer has been completed. If you do not have a signed port form, register the port immediately, print the form, and obtain the customer's port authorisation signature.

Waiting for Form: The Port Authorisation Form has been raised on the Upside web site, but we have not yet received anything.

Incomplete: We have received something, but it is not correct.

Received OK: The form has been received and is correct.
