What do I do with the completed web or manual Port Authorisation Form?
- Ensure that the customer has signed the form and retained a copy. You hold on to the original signed form.
- In ADDITION, you need to arrange port permission from the losing network via ipro or ICMS.
- Once the port is approved, complete the sale and send the ORIGINAL SIGNED port authorisation form to:

- We recommend that you send collected forms to Upside every two weeks, and suggest that you include them in same courier bag as the winback forms, SIM cards and trade-in handsets. Otherwise post them to Upside. Note that Courier Post is the only courier which will deliver to a P.O. Box.
- When forms are receipted at Upside they are marked off against the web site registrations and matched to ICMS / ipro connection records. Non-returned forms are investigated.
IMPORTANT: Send only the ORIGINAL SIGNED port customer authorisation form.
Photocopies, fax, scanned, or incomplete forms will not be accepted.
